We have recently moved and CHIPER’s new office address is 400-500 Portage Avenue, Winnipeg, MB R3C 3X1. As a result, some Harmony applicants or their sponsors may need to submit a revised Clinical Trial Site Information (CTSI) form to update the REB address listed, for trials being conducted under Health Canada regulations.
Health Canada guidance states that if any changes are made to the CTSI form, a revised form should be submitted.
If your CTSI submission to Health Canada included CHIPER’s old address (735 McDermot Avenue Winnipeg, MB R3E 0T6), please be sure you or your sponsor submits a revised CTSI form to update the information in Health Canada’s records. Accurate site details help ensure ongoing regulatory compliance and avoid potential delays in trial initiation or amendment processing.

Revising the REB address for CTSI
The Clinical Trial Site Information (CTSI) form is required to be submitted by clinical trial sponsors prior to initiating a protocol or implementing subsequent amendment(s) at the clinical trial site for trials that are subject to Division 5 of Part C of the Food and Drug Regulations or the Clinical Trials for Medical Devices and Drugs Relating to COVID-19 Regulations.
